By checking yes, I certify that I as the owner am responsible for a full tank of gas, checking my oil, and all other upkeep of my vehicle when dropped off, and I list the problems with my vehicle as honestly and specifically as possible. I also certify that I am responsible fully for the insurance coverage on this vehicle and I hereby agree to hold All Rivers Shuttles, it's drivers and The White Bird Motel harmless of any liability or damage to my vehicle/trailer or personal property. In addition, if there are any extra fees, such as gas/oil  or miscellaneous costs I agree to be responsible for this, and will leave implicit instructions on how that is handled Should there be a major problem with my vehicle, I authorize All River's Shuttles to contact my emergency contact number if there is one provided.  If I should cancel, and let All Rivers Shuttle or The White Bird Motel know via fax, e-mail, phone or note within 48 hours, all monies will be refunded if paid. Should you not notify us within 48 hours or at  your 50% of the total cost will be the responsibility of the vehicle owner and is to be paid to our service. Should any repairs be necessary, we will follow your instructions as per your form.   If triple A is a service your subscribe to or any other automobile emergency group, we will contact them according to your instructions. By signing this form or checking yes I am agreeing and stating that my vehicle being shuttles is fully insured as required by law and that I give permission to drive my vehicle for the purposes of this shuttle.  I understand the person driving my vehicle is a legal, licensed driver in the state of Idaho. We will make every possible effort to provide for the safety of yoru vehicle during the shuttle, however we cannot assume legal responsibility for any vehicle or personal articles including fire, theft or vandalism that may occur while vehicle is parked at a parking lot area or landing, any accidents or for time or cost arising from any indisposition.
