| By checking yes, I
certify that I as the owner am responsible for a full tank of gas, checking my
oil, and all other upkeep of my vehicle when dropped off, and I list the
problems with my vehicle as honestly and specifically as possible. I also
certify that I am responsible fully for the insurance coverage on this vehicle
and I hereby agree to hold Salmon River Services, LLC, All Rivers Shuttles, and
the drivers contracted out by named above harmless of any liability or damage to my vehicle/trailer or personal
property. In addition, if there are any extra fees, such as gas/oil or
miscellaneous costs I agree to be responsible for this, and will leave implicit
instructions on how that is handled. Should there be a major problem with my
vehicle, I authorize All River's Shuttles to contact my emergency contact number
if there is one provided. Should any repairs be necessary, we will follow your instructions as per your form. By signing this form or checking yes I am agreeing and stating that my vehicle being shuttled is fully insured as required by law and that I give
permission to drive my vehicle for the purposes of this shuttle. I understand the person driving my vehicle is a legal, licensed driver
and that they have provided me with that information of which All River's
Shuttle has a copy. We will make every possible effort
to provide for the safety of your vehicle during the shuttle, however we cannot assume legal responsibility for any vehicle or personal articles including fire, theft or vandalism that may occur while
vehicle is parked at a parking lot area or landing, any accidents or for time or cost arising from any indisposition.
I also agree that if I cancel, and not give 24 hour notice, I will be charged
for 1/2 of the fee, unless it is an emergency or the water levels prevent me
from doing so. I am agreeing that..
I Have a Full
Tank of Gas or near full and/or have left some money to have it filled with gas
and or oil, transmission fluid, etc and I have started the vehicle and drove it
more than a minute with my spare key. I have also given the key to the lock on
the trailer and checked the oil, liquids, tires, spares and bearings on my
trailer and/or vehicle. If I have not left a key and need to cut the lock on the
trailer in order to repair it, I agree that I will not hold All Rivers
responsible for this nor charge them for the lock. I will disable the alarm
system or describe the alarm system in detail with instructions to All Rivers
Shuttle so that they may be able to drive it. If I leave a tip, I will mark it
clearly that it is a tip, or I will verbally tell the office that there is a tip
and if there is any garbage to be dumped I will specify it in a note or
verbally, however I will make an effort to dispose of it so that it will not
stink. In case my vehicle becomes inoperable, unsafe or unlawful for any reason
(including but not limited to: engine or mechanical failure, security issues,
ignition or gas cap key problem, no spare tires, I authorize All Rivers Shuttle
to exercise the option on my behalf that I have stated on the form. I authorize
to pay for all reasonable expenses upon receiving my vehicle. I understand that
the roads driven may be rough and poorly maintained and I will not hold All
Rivers Shuttle responsible or their drivers responsible in any way for damage
done to the above vehicle while en route or parked. Permission is hereby granted
to drive the vehicle above and it is covered by public liability and property
damage insurance that will cover the vehicle and shuttle driver. The vehicle has
at least $100 deductible collision and comprehensive insurance. If it does not,
I understand that I am liable for charges resulting from any deficiency in
coverage.
If I choose yes on the form it is the
same as my signature. If I fill the form out for someone else's vehicle
then I am liable for any charges.
Should I cancel, If will do so in the
form of written confirmation, via email or mail or in person at least 24 hours
before my launch date. If I do not, I am subject to pay 1/2 of the charge
except when water level conditions or emergency situations arise. If I do
cancel before 24 hours, and my credit card is charged, you will receive a check
within two weeks after it is processed minus 3.5%. This is what the credit
card company charges. |
